Subcutaneous (incisional) endometriosis is a well described but extremely unusual entity. Patients who remain undiagnosed despite suffering associated symptoms are not infrequently encountered. This case report presents a patient who had an incisional endometriosis that developed on scar tissue following cesarean section. The patient was suffering from an atypical pain on the incision line which was associated with menstrual cycles. A palpable mass that resolved during the luteal phase was detected. An ultrasound imaging and a computed tomography scanning were carried out to delineate the size and relations of the mass. After total excision of the lesion, a histological diagnosis of endometriosis was achieved. During two months of follow-up, there has been no recurrence of symptoms. Scar endometriosis is such a diagnosis that should be kept in mind in patients having a pain associated with incision after any abdominal surgery.
