50mg/m² İM Single Dose Methotrexate Treatment At Ectopic Pregnancy

OBJECTİVE:

To determine the eficacy the magnitude cure relationship of single dose 50mg/m² mothotrexat treatment wich is a modality of medical treatment in ectopic pregnancy as well as anderstanding and finding out the fertility potential in post-methotrexat tretment.

MATERİAL AND METHODS:

98 cases diagnoses to have ectopic pregnancy within the years 1998-2000 in Ankara Zekai Tahir Burak Education and Aplication Hospital of Gynecology.Some anemnesis like the cause of the referral of the cases, the clinical findings, age, gravida,parity and laboratory findings were evaluated.Ectopic pregnancy diagnosis were made in all of the cases by way of ultrasonography and due measurements were made being based upon ectopic pregnancy sac.Single dose 50mg/m² im methotrexat was aplied.After β-hCG become zero the patients were called for Histerosalpingography in order to evaluate the tubal defect level of the treatment in tree months.Besides, the first one years of fertility conditions of the cases during the treatment were evaluated.

RESULTS:

Of the 98 cases, 81 were found to have a cure (82,7%), in 17 of the cases (17,3%) surgical intervention was needed.İn 27 cases who were evaluated in terms of tubal opening in the posttreatment 3 months, 74% (20/27) of tubal opening was detected,in 25,9% (7/27) cases tubal oclusion was detected.When the fertility conditions in the post-treatment one year were evaluated, in the cases being assesed in terms of methotreaxat, this rate for fertility was 91,2%,whereas in the cases undergoing surgical procedure this rate found to be 41,4%.

CONCLUSİON:

50mg/m² im single dose methotrexat treatment in selected cases is very much effective and it is an alternative to the surgical treatment in terms of its cure chance and high fertility potential.
